Nope to #1. At this time they are inside but on the outside of the nether region. They took the long way 'round and ended up on the outside looking in where they started.

Nope to #2. The diaphragm is a barrier between the thoraxic area where the heart and lungs and shit are located. But down in the abdomen where the stomach, large and small intestines, bladder, kidneys, etc, that's all fair game.
